Health Office

Physicals: All students entering the school district for the first time, those entering Pre-K or K, 1st, 3rd, 5th, 7th, 9th and 11th grade, will need a health examination physical. The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner, and presented to school within 30 days of entering school. Physicals are valid if performed no sooner than 12 months prior to the first day of school of the year in which it is required.
 
 
 A Registered Nurse is always available to students from 8:30 a.m. until 4:00 p.m., Monday through Friday. Students needing to see the nurse should ask their teacher to sign a pass,  and then report directly to the health office. 
 
For the safety of your child, any relevant health information should be included in your child's medical file. This information is kept confidential.
 
Any request for an elevator key, or questions regarding health policies and procedures should be referred to the AMS Health Office at 362-7104.
 
If a student is to be excused from Physical Education for one to two days, please send a note to school with your child.  If a student is to be excused from Physical Education for more than two days, a physician’s note is needed. PLEASE NOTE:  THE NURSE DOES NOT EXCUSE STUDENTS FROM PHYSICAL EDUCATION.
 
To control the spread of illness, please keep your child at home if they exhibit any of the following symptoms: 
 
• Temperature in excess of 100 degrees 
• Acute sore throat 
• Nausea, vomiting, diarrhea 
• Inflammation of the eyes with crusting of the eyelids
• Head Lice

Medical Policy

For Prescription Medication
Written orders from the physician must be on file stating: 
 
  1. Student’s name  
  2. Name of drug  
  3. Dosage  
  4. Purpose of the medication  
  5. Time of day medication is to be given  
  6. Number of days it needs to be given  
  7. Possible side effects   
  • The medication must be brought to school in a container appropriately labeled by the pharmacy. 
  • Any prescription medication administration will be recorded in the student's medical file.
  • Medication will be stored in a clean, locked cabinet or container.  Any exceptions (i.e. asthma inhalers/student may self carry) to this provision must be specifically stated by the physician on the written form.  
  • Unless these requirements can be met, medications will not be administered at school.  

For Non-Prescription Medication

Please request the non-prescription form from the school nurse. 
In order to comply with NYS laws and the physician for the Amherst Central School District, the following information is required for non-prescription medications:
 
  1. Written permission from a parent or guardian
  2. The name of the medication
  3. Dosing instructions
  4. Specific symptoms for which the medication is to be given 
  • The medication must be in the original container or package.
  • Acetaminophen, ibuprofen, cough drops, throat lozenges, non-drowsy alcohol free decongestants, antacids and eye drops are the only medications that may be given without a medical provider's written order.
  • Herbal/alternative remedies are not allowed.
